Individuals that are home bound due to immobility or illness, need care on an intermittent basis, and meet one of the following criteria may benefit from HHCP professional services. – New diagnosis of disease process – Exacerbation of a chronic condition – In need of assistance and education to manage medication regime – Open wound or surgical site that requires treatment – Need for Intravenous medication or hydration – Debility, decreased mobility and inability to perform activities of daily living – Pain & Sympton control
