Diabetes Prevention & Control Program – PA Department of Health

The Diabetes Prevention & Control Program supports outcome-based community diabetes initiatives providing services such as weight management, support groups for persons with diabetes, cooking classes, community education, professional education and self-management skills for persons with diabetes. It also addresses diabetes in the disparate populations with initiatives providing services to the African American and Latino populations. Focusing on Clinical Standards of Care, a third initiative of the Department has focused on efforts to improve adherence to these standards for persons with diabetes in the primary care setting.
